Scheduling

Appointments for a consultation with one of our surgeons or specialty departments can be made by calling (541) 930-8900.

Office Hours: Monday – Friday from 9 a.m. to 12 p.m. and 1:30 p.m. to 5 p.m.

Referrals

We will need a referral from your primary care or treating physician to schedule you for an appointment.  If your insurance requires authorization, we will also need a referral authorization from your insurance company.  We do accept self referrals for hernias and hemorrhoids, but the insurance authorization is still necessary when your insurance company requires one. If your insurance company requires a referral to visit our physicians, it is your responsibility to obtain a valid referral through your insurance company. If you do not have a valid referral for your visit you will be responsible for any and all charges incurred at the time of visit.

Medical Records

Oregon law allows thirty days for processing all medical record requests. However, it is our goal to have them processed within 7-14 business days.

Requests from legal establishments, life insurance companies, and copying services will be processed with the appropriate signed authorization from the patient, and after the standard fee has been received.

Patients requesting a transfer of records must submit a written request if the records are being sent to a doctor other than their primary care physician. For your convenience you may either Request Medical Records online or print out and complete a Request for Records Form PDF .

Please note: we are unable to process requests if the form is not filled out completely.

If you have an urgent request, it is recommended that you call our office at 541-930-8900, and ask to speak to a medical records staff person.

Prescription Refills

It is best to call your pharmacy when needing a prescription refill as our office needs at least 3 DAYS notice to refill your prescription. We will not refill narcotics after hours or on weekends.  California residents must fill their prescriptions in Oregon before leaving the state.  You may also request a refill online.
